(1) An application for an order of domestic violence restraining order must be in accordance with Form 1.

(2) The court shall issue an interim domestic violence restraining order against the respondent in accordance with Form 2.

(3) The court shall issue a domestic violence restraining order against the respondent in accordance with Form 3.
